Montero and his team at Cleveland Clinic initiated a quality improvement project in the oncology and palliative care units to reduce readmission by improving post discharge follow-up.

Follow-up included scheduling appointments for within five days of discharge and scheduled nurse follow-up calls within 48 hours of discharge.

Palliative care efforts were also increased. These steps reduced readmissions by 5%.
