Phase 3, Randomized Study of Talquetamab Plus Daratumumab ± Pomalidomide vs Daratumumab Plus Pomalidomide and Dexamethasone in Relapsed/Refractory Multiple Myeloma: MonumenTAL-3

In this OncLive Rapid Readout, Peter Voorhees, MD, of Atrium Health Levine Cancer Institute and Wake Forest University School of Medicine, reviews primary analysis findings from the phase 3 MonumenTAL-3 trial in relapsed/refractory multiple myeloma. The study evaluated the GPRC5D-targeted bispecific antibody talquetamab plus daratumumab, with or without pomalidomide, compared with daratumumab, pomalidomide, and dexamethasone in patients who had received at least 1 prior line of therapy. Voorhees highlights significant progression-free survival improvements with both talquetamab-containing regimens, including strong activity across predefined high-risk subgroups, higher complete response rates, and deeper MRD-negative responses vs standard of care. He also discusses overall survival trends, hematologic toxicity, infection rates, cytokine release syndrome, and GPRC5D-related adverse effects such as taste changes, skin and nail events, weight loss, and balance disorders. The findings support talquetamab plus daratumumab, with or without pomalidomide, as a potential off-the-shelf option for early relapsed myeloma.
