Dr Voorhees on Efficacy Data for Talquetamab-Based Combos in R/R Myeloma From MonumenTAL-3

Peter Voorhees, MD, of Levine Cancer Institute of Atrium Health and a professor of cancer medicine at the Wake Forest University School of Medicine, discussed the key efficacy findings for talquetamab-tgvs (Talvey) plus daratumumab (Darzalex) with (Tal-DP) or without (Tal-D) pomalidomide (Pomalyst) in relapsed or refractory multiple myeloma from the phase 3 MonumenTAL-3 trial (NCT05455320),which were presented at the 2026 EHA Congress.

Voorhees began by laying out how efficacy data from each arm of the trial stacked up with one another. Data from the trial showed that patients who received Tal-DP (n = 287) achieved a median progression-free survival (PFS) that was not reached (NR) vs 24.4 months with daratumumab plus pomalidomide and dexamethasone (DPd; n = 290), translating to a 72% reduction in the risk of disease progression or death (HR, 0.28; 95% CI, 0.20-0.40; P < .0001).

Furthermore, the median PFS with Tal-D (n = 287) was also NR, which ultimately resulted in a 67% reduction in the risk of disease progression or death vs DPd (HR, 0.33; 95% CI, 0.24-0.46; P < .0001).

Reductions in the risk of disease progression or death with both Tal-DP and Tal-D compared with DPd were both statistically significant, Voorhees said. He continued to outline the benefits of Tal-DP and Tal-D, noting that survival benefits were observed in clinically relevant subgroups of patients in the trial. Patients who were older, had previous daratumumab exposure, had stage III International Staging System disease, had high-risk cytogenics, or had soft tissue plasmacytomas all experienced PFS advantages when treated with Tal-DP or Tal-D vs DPd, he explained.
