However, practical implementation presents challenges regarding the continuous treatment approach rather than the traditional induction-maintenance paradigm that provided patients with a defined endpoint and psychological benefit.

Dr. Kang initially questioned the study design's continuous therapy approach, being entrenched in the taxane-HER2-pertuzumab followed by maintenance model. However, she now recognizes that the mixed population of HER2-positive advanced disease includes patients requiring sustained intensive therapy for optimal outcomes, validating the study's design demonstrating continued ADC-based chemotherapy benefits. The challenge involves balancing T-DXd's proven efficacy against long-term toxicity considerations, particularly fatigue and gastrointestinal effects that can significantly impact quality of life.

Maintenance strategy optimization incorporates data from HER2CLIMB-05 and PATINA trials, which demonstrate enhanced PFS through adding tucatinib or combining endocrine therapy with palbociclib to trastuzumab-pertuzumab maintenance. Dr. Kang emphasizes that no patient should be denied these intensification approaches if tolerable, noting that palbociclib has established tolerability while tucatinib carries additional risks including grade 3 liver enzyme elevations and diarrhea. The paradigm shift eliminates rigid cycle counting, favoring individualized approaches based on patient response, scan results, and performance status improvements.
