The faculty describe TP53 as one of the most common mutations in cancer, which can present clinically as higher tumor mutational burden and more advanced or aggressive disease. They explain why exploratory subgroup analyses from FLAURA2 and MARIPOSA, in which most enrolled patients carried some poor prognostic risk factor, suggested benefit from escalation without establishing it definitively. The TOP study prospectively randomized patients with concurrent EGFR and TP53 mutations to osimertinib plus carboplatin and pemetrexed or osimertinib monotherapy, and the panel notes the findings closely mirrored the earlier FLAURA2 data, supporting TP53 as a negative prognostic factor that can be influenced by frontline chemotherapy addition. A stated limitation is that TOP did not subset TP53 alteration types.
