The analysis highlighted methodological challenges with median treatment duration exceeding 43 months for zanubrutinib versus 13.8 months for venetoclax plus ibrutinib combinations. Despite longer exposure, zanubrutinib showed better safety advantages in diarrhea, neutropenia, nausea, anemia, and atrial fibrillation. Dr. Shadman explains this apparent contradiction through cumulative risk analysis: well-tolerated drugs continued longer maintain low cumulative risk despite extended exposure, while combination therapies require intensive monitoring for cytopenias, gastrointestinal toxicities, and treatment interruptions. Patient-reported outcomes and quality-of-life considerations during combination therapy represent important factors not fully captured in safety analyses but crucial for clinical decision-making regarding treatment tolerability and patient experience.
